Care Coordination is serious about the quality of it services and in continuous improvement. In order to ensure our services meet the needs of those who use them we carry out regular satisfaction surveys. Every month a proportion of all clients receiving a needs assessment is surveyed and every 2 years a referrer/provider survey asks the opinions of health professionals, funders and others who have contact with our service. The responses are discussed by the team and any necessary changes or improvements made and evaluated. The most recent survey results can be found in the news section of this website.
