The Canterbury Care Coordination Centre (CCCC) has been operating since early 2004. Our team of Registered Nurses and administrators manage, on average, 500 referrals a week for community based health services in the Canterbury area.

We manage referrals for people of all ages for DHB funded community-based services, including:

  • District Nursing.
  • Short term Home and Community Support Services (HCSS)
  • Specialist Nursing Services e.g. continence, stoma,
  • Other services e.g. specialist palliative care, medication management, falls prevention

We also forward referrals for non-CDHB funded services to the appropriate service or region where possible. In broad terms, we work with referrers to identify the level and type of service required, with the DHB to ensure eligibility and with providers to support and facilitate the provision of care.

In addition to referral management our administrative functions support invoicing and payment for a range of services and also detailed reporting, providing funders and others with key information about community based health care in Canterbury.

Care Coordination is staffed from 8.30 - 5pm weekdays and 8.30 - 4pm weekends and public holidays
