Single Point of Entry

Care Coordination was designed specifically to provide a single point of entry into that system, literally coordinating all the health and support needs of the client through one agency.

The range of services offered through Care Coordination can be tailored to not just individual clients, but the services and support available in specific districts. These restorative packages of care deliver services into a client’s home in a way that is flexible enough to meet their needs and achieve their goal of being as independent as possible.

Meeting the goals of better health and greater independence for clients is the foundation of restorative care. By working in partnership with the client, their family/whanau and health and support network, the care they receive can be tailored with those goals in mind.
